Getting Ready for Your Appointment
A little preparation helps your visit go smoothly. Complete your forms online ahead of time so we can check you in almost instantly. Forms are available in English and Spanish. Please bring a photo ID, your dental insurance card if you have one, and a list of any medications you take.

We believe cost should never keep you from the care you need. Belmont Dentistry is in-network with many PPO dental insurance plans. Our team will verify your benefits, answer your questions, and work to maximize your coverage so there are no surprises.
If you do not have insurance, our in-house membership plan is a convenient way to save on your dental care. Larger treatments can be made more manageable through CareCredit, which offers low or no interest payment options.
